Please fill out and submit this online Warranty Service Request form or print and mail/fax the printable document (pdf file). All warranty requests for service need to be sent by mail (to address below), fax (to 269-345-1570) or via this online form. The more detailed the explanation to the problem and its location, the better prepared we will be upon arrival to your home. You will be contacted to schedule a day and time to visit your home to make the necessary corrections covered under the warranty period.
Some items may need to be ordered and can take up to 4 weeks for delivery so we appreciate your patience for those ordered items. All warranty work will be done between 8am and 5pm Monday-Friday. The homeowner must be present while work is being done. If you have questions about your warranty please consult your Homeowner Manual or the Warranty section of this website.
